Notes:

Length: 4.25mm. Eyes hairy; clypeal margin with two tiny teeth; genal carina meets hypostomal carina some distance from mandible base.

Martin Schwarz says (from the photos): keys well to Phygadeuon rotundipennis in Horstmann (Entomofauna 14: 85-148, 1993), but comparing your specimen with specimens in my collection I am not sure if it is this species. There are some more or less slight differences.
